Thank you for volunteering to complete our survey – we truly value your comments, ideas and thoughts.
You are receiving this survey because you have indicated you are happy to share your ideas and comments, or because a friend believes you would be interested in giving your views and has forwarded the link on to you – either way – thank you.
We are a team of people from two organisations: ACT 4 and Wildstone PKG. We are are passionate about helping young people keep safe and make good choices in the moment so they can enjoy healthy, fun, and rewarding relationships.
About the survey
As part of our work to create programs and resources to support young people to navigate (the sometimes complex) pathway of having healthy relationships, we are developing a new ‘game-like’ product for schools.
It will be a unique, scenario-based activity, with game-like features, designed to develop knowledge, skills and mindsets, helping young people discover and explore their attitudes and behaviours in a safe, age-appropriate way.
We want to ensure we create something that is relevant and meaningful – and that is where you come in! We would love to hear your honest and candid thoughts and ideas – however ‘out-there’ you think they are.
Please give as much information as you can.
The survey will probably take you about 20-25 minutes to complete. You will receive code at the end of the survey that you can use to claim your thank you gift voucher.

Safeguarding you and your privacy
Your responses will be stored securely on the act4 server and will only be used for the purpose of informing the development of the product, including the development of relevant scenarios.
The responses you share will be confidential, we will not identify any individual comments and all information will be combined and anonymised.
We do however want to be able to develop age-appropriate resources tailored to schools, we do ask for your age and the type of school you attend. This will only be used to collate the ideas and comments into the relevant age groups.
Throughout the survey, we are not asking you to share any personal information about yourself, however, we would like to understand your thoughts and ideas about the different scenarios you and your peers find yourselves in.
If you feel uncomfortable at any time, take care of yourself by exiting the survey, and reach out to someone you trust to support you.
